I, the undersigned, hereby grant Jeremy Ito Story Design, LLC and its clients the right and permission to use my name, image, likeness, voice, and performance in any video, photograph, audio recording, or other media recording taken of me or in which I appear (collectively, the “Materials”) for marketing, promotional, or educational purposes.

I understand that the Materials may be used in a variety of media formats, including but not limited to print, digital, social media, and broadcast. I also understand that the Materials may be used in perpetuity, and that I will not be entitled to any compensation for such use.

I hereby release and discharge Jeremy Ito Story Design, LLC and its clients, their officers, employees, agents, and successors from any and all claims, demands, and causes of action whatsoever arising out of or in connection with the use of the Materials, including but not limited to any claims for invasion of privacy, defamation, or infringement of any right of publicity or copyright.
